Community Nursing
Community Nursing Care aims to support your independence and your lifestyle in a safe and familiar environment. We provide clinical and personal nursing services to members of the community in the comfort of their own home.
Life Stage Transition
We focus on short and long-term supports that focus on strengthening the participant’s ability to coordinate their supports, and to assist them to live at home and participate in the community.
Community Participation
My Community Care provides environments that will allow you to participate in the community base programs. My Community Care is committed to finding you the best community base activities.
Transport
We provide transportation services at your doorstep to help you travel around the community, attain appointments and social outings. We also provide right equipment and transportation services based on your requirements.
Household Task
We perform household tasks of your choice to keep your home neat and clean. Our experienced and friendly team will provide you with daily chores and facilitate your independence in day-to-day life.
Personal Activities
We are committed to providing personal activities that are aimed at making your life comfortable and easy. Our well-trained staff are passionate to deliver care that is suitable to you. We promote independency and choice of control.
Support Coordination
We provide support coordination level one and two. We will help you to get more from your new NDIS Plan or your current plan.
Home Care Services
My Community Care provides home care services to people experiencing ageing. Home Care Packages (HCP) are one of the ways that older South Australians can access affordable care services to get some help at home.
Outing
My Community Care offers a range of outing services for people with disabilities to help them participate in community activities and lead a fulfilling life.
