Job Responsibilities
- Personalized Care: Provide high-quality care and support to individuals with disabilities, ensuring their physical, emotional, and social well-being.
- Individual Support Plans: Implement and follow individual support plans based on the unique needs and goals of each individual, as outlined by their healthcare professionals and/or guardians.
- Daily Living Assistance: Assist with personal hygiene, grooming, dressing, meal preparation, feeding, and other activities of daily living.
- Medication Administration: Administer medications according to prescribed schedules and document them accurately.
- Mobility Assistance: Support individuals with mobility needs, including transfers, wheelchair assistance, and ambulation.
- Emotional Support: Offer emotional support and encouragement, promoting positive mental health and overall happiness.
- Social Inclusion: Organize and facilitate community outings, social activities, and recreational events to promote social interaction and community integration.
- Documentation and Reporting: Maintain accurate and detailed records of daily activities, progress, incidents, and observations, adhering to the organization and legal requirements.
- Communication: Collaborate effectively with individuals, their families, healthcare professionals, and other team members to ensure a holistic and person-centered approach.
- Safety and Crisis Management: Identify and mitigate potential risks, respond to emergencies, and follow safety protocols to maintain a secure environment.
